How to Login:
- Enter the Email Address that you gave us. If we don’t have the exact email address on file, you will need to contact us.
- Stuck? Text us at 408-739-6200 for help with your email address to ensure we have the right one.
- First time logging in? Click “Not Registered”
Step 2 – Registration
If this is your first time, you will need to register.
How to Login:
- Enter the Email Address that you gave us. If we don’t have the exact email address on file, you will need to contact us.

Step 3 – Validation
- To register, you will need a Validation Code.
- The code is sent by TEXT MESSAGE to the cell phone we have on file.
- Enter your Birth Date.
Can’t validate your details? Text us at 408-739-6200 to verify your:
- Cell phone
- Date of birth
If our records do not match, you will not be able to access your portal.
After your in, Navigation is simple
The portal has two sections.
1. My Health Records
- Our letters
- Pathology reports (if requested)
- Lab Reports (if requested)
2. Message My Provider
- Please do NOT USE THIS – as we won’t get your message
- Instead, text us at 408-739-6200
Once you enter one of these three sections, you can return to the main navigation bar by clicking on the “home” icon
